Rachael Sood, RN, MSN, NP-C, CDCES
Rachael is a Diabetes Nurse Practitioner, born and raised in Lacombe, Louisiana. She is a local, small business owner in St. Tammany Parish and is passionate about caring for the community in which she lives.
She is a proud 4th generation Registered Nurse (RN) and is the daughter of Mrs. Michelle Gorney, an RN in Slidell, Louisiana. She is the granddaughter of the late Mrs. Lin Holmes, RN, and the great granddaughter of the late Mrs. Philomene Holmes, RN. Nursing Experience
Rachael began her nursing practice in 2006 as a Cardiac Intensive Care Nurse (CCU) after receiving her Bachelors of Science in Nursing degree from Southeastern Louisiana University in Hammond, Louisiana. She worked locally as a staff nurse in the ICU's of Louisiana Heart Hospital, St. Tammany Parish Hospital, and finally Tulane Medical Center in downtown New Orleans. Her passion for patient care grew, and she returned to school determined to arm herself with tools to make a difference and do more. She graduated from Southeastern Louisiana University earning a Masters Degree in Nursing and an Advanced Practice Registered Nursing Degree in 2011. It was then she began her practice as an Adult Health Nurse Practitioner in New Orleans, Louisiana. For the past 14 years, she has practiced as an NP within the areas of Neurosurgery, Spinal Care, Primary Care, Population Health, Endocrinology, and Diabetes. Over the past 9 years, she has become well-known and respected locally for her expertise and erudition in Diabetes care. At state and national levels, she serves as an expert clinician and public speaker for patient advocacy, empowerment, and Diabetes tech innovation. Rachael is a Nursing leader by advocating for legislation which advances the nursing profession, thereby supporting increased access to care for the residents of Louisiana.
Following the Covid Pandemic, Rachael and her husband moved from Uptown, New Orleans to be closer to family and raise their children here in St. Tammany Parish. Their family has always been at the core of their values and priorities. Rachael quickly realized the region was growing, and there was nearly a 30% increase in the Parish population in just a few years! Hence, a growing and increased need for specialty healthcare access had been born. The Journal of Diabetes & Metabolic Disorders cited Diabetes as a major global health concern in 2024, with a higher prevalence in the United States compared to other countries. Within the U.S., Louisiana is ranked as one of the unhealthiest states to live within. A recent business and market analysis revealed decreased and limited access to focused, advanced diabetes care, as well as a lack of Endocrinologists in the area. The Diabetes Collective was then launched in 2024 with the support of family, collaborating physicians, and the Medical community. TDC's business model acknowledges patients as their own leaders and partners in the development of a Diabetes Self-Management Training plan (DSMT), thereby empowering them to lead healthy and fulfilling lives. TDC's Vision is that of being a change agent in redefining the way Diabetes care is seen and delivered.

The Holmes Family Medical Legacy
Rachael is a 7th generation health care provider. She was born from a family deeply rooted in medical care. As a young child, she often accompanied her mother to work at the hospital. She watched as her grandfather stayed awake for days on end, going out for emergency operations in the middle of the night, and delivering babies that came on their own time. Growing up within the turquoise-painted halls of waiting rooms, the constant ringing of the hallway telephone, listening to the telemetry monitors beeping....eventually became this familiar comfort. She was surrounded by patients, nurses, doctors, and a sense of togetherness. The work ethic, joys, and hope for a healthier tomorrow luckily got passed down.
This office is more than just a Diabetes care clinic; it is the continuation of the Holmes family medical practice, rich in ethical patient care that began in the early 1800’s. Our family is vested in the community and is passionate about serving others. You are not just a number or a patient here – you are family!

Traveled to North America in 1737 from Antrim, Ireland, and was a trader of goods with the American Indians in the states of Georgia and South Carolina. He had several heirs. One of his daughters, Barbara, was known to be very studious and educated. She eventually married William Holmes, and they had a son, Thomas Galphin Holmes.

The son of William and Barbara Holmes, and the Grandson of George Galphin. He moved from South Carolina to Alabama and served as a General Physician and Surgeon in the U.S. Army. His academic education is known to have been in old field schools under private tutors. He was one of the sole survivors of the Fort Mims Massacre of 1813, by the Creek Indians where over 500 people were slaughtered. He practiced medicine for 52 years. He was a founding member of the Montgomery Hill Baptist Church, in Tensaw, Alabama. He was married to Elizabeth Weekly, and they had 8 children. The 8th child was Origen Sibley Holmes, named after a best friend.

He served as a General Physician and Assistant surgeon in the confederate calvary of the civil war for 4 years. He served as a House Representative in the Alabama State Legislature in 1872, and practiced medicine for 30 years. He was married to Nannie Boyles, and they had a son, Origen Sibley Holmes, II (otherwise known as Dr. Sibley Holmes II).

A Family Doctor who graduated in 1895 from medical school. He was sent to Honduras by the U.S. Military to help fight Yellow Fever, then was selected as a division surgeon to the Mexican railroad, near Torreon, Mexico. He practiced in Houston, Texas shortly after, then moved to settle in Alabama, where he served as a family physician. Dr. Sibley was an advocate for elementary, secondary, and vocational education. He was instrumental in developing the school systems in South Alabama, where he served as Chairman of the Board of Education in 1916. Then, like his father, served as a Senator in the Alabama Legislature in 1919. He was married twice. His first wife Irene Williams died in 1901 during childbirth, having their 3rd child, Williams Coghlan Holmes or fondly referred to as “W.C.”. He then remarried her sister, Maiben “Nanny” Williams.

The son of Dr. Sibley Holmes. He was a Medical Doctor and Surgeon and brought the first adequate medical facilities to Baldwin County, Alabama by opening Sibley Holmes Memorial Hospital in 1936. He married Ms. Philomene Boogaerts; they met working together at Schumpert Memorial Hospital, in Shreveport, Louisiana while she was in nursing school. She became an RN, who eventually ran their hospital and medical practice while he was away at war. Dr. W.C. served in the U.S. Navy as a commander and Senior Medical Officer in 1943, during World War II. At this time, there were no "Nurse Practitioners" and the "scope of advanced nursing practice” didn't yet exist. Philomene was a modern-day Nurse Practitioner though, delivering babies, administering anesthesia, performing surgeries, prescribing medicines, and doing house-calls by horse and carriage. Many of the older members of the community will tell you that she was “as good, or better of a doctor than her husband”. She was a hometown hero, being the only nearby source of medical care. Her Nursing Practice was precedented 20+ years prior to the actual first labeled “Nurse Practitioner”. Dr. Holmes later passed away from ALS in 1961. Mrs. Holmes carried on his practice, eventually precepting and helping train some of the first known NP's in the state of Alabama. The hospital remains standing to this day in Foley, Alabama, and is known as the Holmes Medical Museum. A bridge was then built over the intercostal canal in Gulf Shores, Alabama in his honor – “The W.C. Holmes Bridge”.

She can be described as a hard-working, tunnel-visioned, and very "direct" woman - especially strict with her boys having proper manners.

Dr. "Jim" Holmes is the son of Dr. and Mrs. W.C. Holmes. He trained for medical school and Residency at Tulane Medical School in New Orleans, Louisiana. He met his wife, Ms. Lin Palmer Holmes on a blind date while she was in nursing school rotating at Montelepre Hospital. A good friend told him of a a "new town near New Orleans" named Slidell. This community was growing rapidly, and they needed help with obstetrical care and delivering all those babies! Dr. Holmes bought a plot of land and had an office built near what is now known as Slidell Memorial Hospital. He practiced as an OBGYN for over 40 years. He is well known for his love of gardening and the outdoors, producing award-winning tomatoes and Camellia flowers. He is an avid fisherman and enjoys being out on the water and boating. Dr. Holmes is now retired, but still resides in Slidell, spending time at his church and with family. Fun facts about Dr. Holmes: Has lots of great stories, and there's not one thing he can't "fix". He recently celebrated his 90th birthday surrounded by family and friends.

Originally from a town near Starkville, Mississippi. Lin attended Spring Hill Nursing school in Mobile, Alabama, and did clinical rotations at Montelepre Hospital in New Orleans, Louisiana. It was there she met her husband, Dr. James Sibley Holmes, on a blind date. Shortly after they were married, and moved to Slidell, Louisiana where Dr. Holmes opened up his own Obstetrical practice. Lin loved to socialize, host parties, play tennis, and sew/Quilt. She also volunteered her time within multiple churches and community organizations. Lin had 6 children; the 2nd child was Michelle, who later became an RN like her. She is the grandmother of Rachael Sood, NP. Lin passed away from pancreatic cancer in 2016. I can still hear her laughter and remember so many words of wisdom. She would be so proud of all of us, and know she is looking down from Heaven. Not a day goes by that I don't miss seeing my phone ring with the caller ID: "Mimi".

The daughter of Dr. and Mrs. James Sibley Holmes, from Slidell, Louisiana. She recalls even from a young age, that she wanted to be a nurse like her mother and grandmother. Michelle graduated as an RN with a Bachelor of Science in Nursing from the University of Louisiana at Lafyette. She married Mr. Charles P. Gorney, Jr. of Slidell, Louisiana and moved to Lacombe, Louisiana. They have 3 children: Rachael, Paul, and Philip. Michelle has over 40 years in nursing experience within ICU, Operating Rooms, PACU (Recovery Room), and Hospice Care. She is a well-known community leader like her mother giving time to non-profits and volunteering at her church. Her special Legislative efforts have included advocacy at local and state levels for Increasing access to care for Women during pregnancy and increasing Rights for Unborn babies. Michelle proudly supports her daughter Rachael in her nursing endeavors.

She is the daughter of Mr. and Mrs. Charles P. Gorney, Jr. of Lacombe, Louisiana. Her academic journey includes an RN and Bachelor's of Science in Nursing as well as Master's of Science in Nurse and APRN, Adult Health NP degrees from Southeastern Louisiana University in Hammond, Louisiana. Rachael has 18 years of Nursing experience and direct patient care within St. Tammany, Jefferson, and Orleans Parishes within hospital and outpatient settings. She has nursed in the ICU, CCU, and transplant units as an RN. Her experience as an NP has been within Neurosurgery, Primary Care, Population Health, Endocrinology and Diabetes. She has served on ethics boards for hospitals, an NP preceptor, a community educator, and is a public speaker on Healthcare access and patient empowerment at state and National levels. She is the Owner and Operator of The Diabetes Collective, the first and only clinic dedicated to Diabetes care in St. Tammany Parish. The vision of her clinic is to redefine the way Diabetes care is seen and delivered. Rachael resides in Mandeville, Louisiana with her husband and children. In her spare time, she likes exercising, good music, eating at Local hot spots, and being outdoors - especially the beach!
